Making an Appointment
To make an appointment, you can either call us at the number listed above or fill out the form on our Request an Appointment page. Emergencies are seen during regular business hours, and our staff monitors our call center and regularly returns messages during non-business hours. Please help us serve you better by keeping your scheduled appointments. You must give the office 48 hours notice for cancelled appointments or there will be a charge applied to your account. Should you have any questions or concerns, please do not hesitate to contact us.

We accept Cash, Visa, MasterCard, American Express, Discover and Debit credit cards as well as personal checks. We accept many major insurance plans, including Superior, Davis, Spectera and most VSP. We also accept many medical plans like BCBS, Aetna and United Health Care, which are listed below. If your insurance is not listed, please call us and ask because we may accept it.
